Graduate Student Forms

GRADUATE PROGRAM TIMELINE

All theses and dissertations must be submitted electronically. Please note the deadlines below that pertain to your desired graduation date. If you have applied for graduation and find you will not be able to meet one of the deadlines, you must notify the Graduate School prior to the deadline or your file may be placed on inactive status. Email: gradinfo@uga.edu.

Graduate students must register for a minimum of 3 hours in at least two semesters per academic year (Fall, Spring, Summer), including the 3 hours of graduate credit required during the semester in which degree requirements are completed. PH.D. STUDENTS

  • Advisory Committee Form
    Please contact Angie Stockton (angie1@uga.edu) when you have the names that you want for your committee. This should be done during your first year of graduate work. Angie will submit your Advisory Committee Form to the Graduate School. It must be filed before the end of your first year of coursework. Please read page 13 of the Graduate Student Handbook for rules on setting up your committee.
  • Program of Study
    Ph.D. students must submit a Program of Study Form for approval by the dean of the Graduate School by the time the notification of the preliminary oral comprehensive examination is given. Angie Stockton will submit  your Program of Study form to the Graduate School.
  • Format Check
    Before defending your doctoral dissertation, you must submit one complete copy of your thesis or dissertation for a format check with Graduate School. 
  • Electronic Thesis and Dissertation (ETD) Submission Approval Form
    Submit this form after your defense.
  • Approval Form for Doctoral Dissertation and Final Oral Examination
    Angie Stockton will assign this form to your committee before your defense so they can sign off on it.
  • Application for Admission to Candidacy
    This form must be signed by your Major Professor and the Graduate Coordinator and filed with the Graduate School. You need to initiate this form through GradStatus after your Oral Comprehensive Exam.
  • BMB Mentor-Mentee Form
  • Annual Meeting Form
  • BMB Graduate Bulletin
  • BMB Guidelines and Qualifying Exam Rubric
  • Advisory Committee Form – please email Angie Stockton (angie1@uga.edu) a list of your committee members and she will submit it to the Graduate School. Please read page 13 of the Graduate Student Handbook for rules on setting up your committee.
  • Comprehensive Exam Announcement
    This form must be filed by the Graduate Coordinator Assistant.
  • Dissertation Defense Announcement
    The Graduate Coordinator Assistant will file this form through the Grad Status website
